Every summary I get has a paragraph about something that that was not discussed in the meeting

In every one of my summaries the following paragraph shows up 

Improving Project Structure and Communication

This is never part of the meeting, I am curious if anyone else has this happening.

I've been getting the same thing. At first, the meeting summaries were super accurate, but the last several always include this same bit: 

Improving Project Approach and Efficiency

I've been getting these exact paragraphs too for all of my meetings.  I have found the summaries excellent up until now, but these is a bit suss...
