Can this plan be for an individual who isn't in a business and wants a home office phone for people to call during work hours?
I own a VoIP IP phone - Yealink T46s with locally hosted FreePBX. I wanted to finally switch to a cloud services provider that would allow my "office" or "work" phone to be active 24/7 through the cloud so anyone could call me using my work number rather than personal.
Please let me know if this plan is what I should choose for my needs.

I think Zoom Phone System - US & Canada Unlimited Plan would work for you.
You mention that you want people to be able to call during work hours. Zoom auto receptionist supports both 24/7/365 call routing as well as custom work hours routing. During your business hours, zoom could route calls to your desk phone, your pc and or your cell phone. During non-business hours, zoom could route calls to voice mail and send a transcript and recording of the voice mail to your email account.
With over 7 million active zoom phone seats, Zoom has extensive features that any individual or enterprise user might need.

You will be able to access zoom phone from a windows or a mac pc, ios or android cell phone as well as certified desk phones.
Your Yealink T46s is on zoom phone certified list but must be one manufactured on or after 2019. In addition, if the phone is registered to another voip service, the phone must be removed from the prior service and reset to factory settings.
“Yealink devices manufactured before 2019 are not compatible with Zoom Phone encryption and certificate security requirements. The device will fail Zoom Phone provisioning, even if it was added to the account manually or in bulk.”

Can this plan also allow my "co-worker" and I to both be able to call outbound numbers under 1 plan or does he need to purchase a plan too?
My goal is to have the "Auto Recep" take an outbound user to the menu and they have to press 1 for me and 2 for my co-worker. Once the outbound caller chooses whoever, it calls that extension the phone is assigned to. And if either me or my co-worker want to call out to someone we can do that using the "main" phone plan, but we couldn't call out simultaneously. I also want to call him through an extension, which I know is possible.

Anyone that needs to call outbound numbers needs a calling plan. your outbound calls and your co-worker outbound calls can show your main company number and name.
You and your co-worker can call each other’s extension.
You can have your auto receptionist route incoming calls to your interactive voice response system.

You can use your interactive voice response (ivr) system to provide options to connect to:
you, press one
your co-worker, press two.
Also, there is an option to allow callers to navigate the ivr by voice. Instead of press one, the caller could use your name.

i suggest contacting zoom sales.
for two zoom phone unlimited us/ca calling plans, zoom would normally charge a credit card monthly or you could prepay for a year.

if your colleague has a business with a different name, he/she should probably have a separate account.
as an alternative to a separate account, zoom does permit an account to have multiple sites and each site can have its own auto receptionist, caller id number, and caller id name.
