After providing a registration link for my meeting; when I click on the link my name and email information comes up. How can I have a blank registration form for the members.

That is likely just your computer auto-filling the form based on your previous form submissions. When someone else opens that registration site, they will not see your info. 

To test this, open that registration page in an incognito window. This will open the registration page without any of your previous info and will be how the page appears to everyone else that opens the page.
