Let's say Nancy registers for my Zoom event. As an attendee, I can then designate her to be a Speaker which adds the speaker label to her card, when she shows up on the People tab. However, it seems like Nancy also has to create a separate Speaker profile? Can these two profiles sync?

Not really – they are two very different things.
The Attendee profile is primarily used for networking in the Lobby and Expo in Zoom Events. The Speaker Profile is used to provide information in the Sessions listing, Itinerary View, and featured sessions in the Lobby.
