I currently have a congregation Zoom account that was obtained through the organization and wants to avail/convert to a zoom discounted license. Where could I get password the be able to fill out the online request form?

This appears to be a password used by “the congregation secretary” to access the watchtower.formstack.com folder and associated forms. I believe you need to contact your organization for this.
