Getting CEU credits

I took a class last night. When it was over I was unable to access the evaluation form. I cannot get credit for the class without filling out the form. How can I access this form?

It's unlikely that the form was provided by Zoom.  You should contact the meeting host, the instructor, or the institution who sponsored the course for more information.
