Scheduled a meeting using time slots, but no code is being generated in the email

I'm troubleshooting a problem I've recently been having: When a client picks a timeslot on my scheduler, they get a confirmation email for the meeting but NO code is generated for the future slot. It just started happening in the past couple of days....
