Zoom keeps freezing, and basically not working on continuity camera since latest OS update, I have the latest Zoom update on Mac Desktop.
The camera will keep cutting out zooming in and out and freezing. This is rubbish.

((knock on wood)) I've been having success using continuity camera doing the following:
Make sure wifi and Bluetooth are turned on on both devices and iPhone and computer are on the same wifi
Before you mount your iPhone make sure the screen is black. Do not mount or try to use with the phone unlocked.
I've been connecting my phone to my computer USB-C to USB-C
Hope that helps! For me the biggest difference was having my screen locked and not open before switching to continuity camera.
