Participant showing up twice as attendee

We have a weekly team meeting and everyone has an invite. I have breakout rooms automatically assigned to our smaller teams after we do a quick all team huddle. There is one member who for some reason shows up as an attendee twice, even though she's only logged in once, so she never goes to her assigned room. We have to manually add her to a meeting room each week. Is there something we can change?
