My mom's telehealth assessment can be done with the assessor using Zoom for Healthcare. Does it work if she just has the free basic (i.e. unpaid) Zoom, and is that secure then, in the way that Zoom for Healthcare is supposed to provide?

Yes, if the provider is using Zoom for Healthcare (Zoom provides a Business Associate agreement) any connections they make even to free users provides the security Zoom provides. This means the provider could connect directly to your mom or to multiple users at once (a multi-point call) and they are all covered even if they are on free license. If the provider has the paid license that's what counts. Good question.
