Surveying /Polling Zoom users for Telehealth

I help manage our telehealth program and would like to set up a quick survey or poll at the end of each teletherapy session to receive feedback from both the patient AND the THERAPIST.  I am having difficulty finding a way to survey/poll the therapist on there experience with the telehealth session.  Any suggestions?

Greetings, if the provider is not the host, they will also receive the survey.  So, both the patient and the therapist will receive the SAME survey.  If the therapist is the host of the session only the patient will receive the survey.  Here is some more information on the Zoom survey tool:  More specifics can be found on our support site. https://support.zoom.us/hc/en-us/articles/4404939095053-Enabling-meeting-surveys
